Key Responsibilities
- Serve as the primary point of contact for patients enrolled in chronic‑care programs, ensuring timely communication and follow‑up across phone, email, and portal channels.
- Coordinate care plans with clinicians, pharmacists, and social‑service partners to address barriers to adherence and promote positive health outcomes.
- Conduct intake assessments, verify insurance eligibility, and update electronic health records with accurate patient information.
- Track and report key engagement metrics, such as outreach volume, appointment completion rates, and patient satisfaction scores, using the company’s analytics dashboard.
- Facilitate virtual group education sessions and one‑on‑one coaching calls, tailoring content to diverse cultural and linguistic needs.
- Identify opportunities for process improvement, escalating escalations, and collaborating with quality‑improvement teams to refine engagement workflows.
Required Qualifications
- Bachelor’s degree in Nursing, Health Administration, Public Health, or a related field.
- Minimum of 2 years of experience in patient outreach, care coordination, or health‑service support roles, preferably within a large health system or health‑technology company.
- Proficiency with electronic health record systems (e.g., Epic, Cerner) and patient‑portal platforms.
- Strong verbal and written communication skills, with the ability to convey complex medical information in clear, compassionate language.
- Demonstrated ability to manage multiple priorities, meet deadlines, and maintain confidentiality in compliance with HIPAA regulations.

- Experience with telehealth platforms and virtual care delivery models.
- Bilingual proficiency in Spanish or another widely spoken language.
