Key Responsibilities
- Deliver personalized, evidence‑based health coaching to members via video, phone, and secure messaging, helping them set and achieve goals related to weight management, diabetes prevention, hypertension, and overall wellness.
- Conduct comprehensive health assessments, develop individualized action plans, and monitor progress using Omada’s digital platform and data dashboards.
- Provide education on nutrition, physical activity, stress management, and behavior change techniques, adapting strategies to each member’s cultural and socioeconomic context.
- Maintain accurate, timely documentation of member interactions, outcomes, and any escalations in accordance with HIPAA and company policies.
- Collaborate with interdisciplinary teams—including dietitians, psychologists, and data scientists—to continuously improve program content and member experience.
- Participate in regular training, peer‑review sessions, and quality‑improvement initiatives to uphold coaching standards and stay current with clinical guidelines.

Required Qualifications
- Bachelor’s degree in Health Education, Nutrition, Public Health, Nursing, Exercise Science, or a related field.
- Minimum of 2 years of experience providing health coaching, wellness counseling, or chronic disease management, preferably in a virtual or telehealth setting.
- Certified Health Coach credential (e.g., NBHWC, ACE Health Coach) or equivalent professional certification.
- Strong knowledge of behavior change theories, motivational interviewing, and evidence‑based lifestyle interventions.
- Excellent verbal and written communication skills; ability to build rapport and motivate diverse populations remotely.
- Proficiency with digital health platforms, EMR/EHR systems, and basic data reporting tools.
- Eligibility to work in the United States and reliable high‑speed internet connection for remote work.

Desired Skills
- Experience with chronic disease programs such as Diabetes Prevention Program (DPP) or hypertension management.
- Additional clinical credentials (RN, dietitian, exercise physiologist).
- Familiarity with analytics dashboards and outcome measurement.
- Multilingual abilities, especially Spanish, to serve a broader member base.
- Demonstrated ability to work independently while contributing to a distributed team.

Compensation & Benefits
- Annual base salary range: $70,000 to $95,000, commensurate with experience and qualifications.
